Trying to install ngx-60 hfa-05

Hello, i performed hfa on secureplatform box. everything went well, rebooted, but it still says it is on hfa-04 ?

When i go into smartupdate, it lists hfa04 and hfa05 ? but when i run hfa_ver.sh

this is the output:

So i'd guess to say it was not succesful. Even though at the end of the install from CLI, it said it was. I have rebooted numerous times, still nothing?

Any ideas where to look?
