This seems to be a problem with using the smartcard and R60 - HFA2 and below.
I would disable the smartcard.
Install HFA3. (I've not been able to get HFA4 to install yet!)
Enable the smart card.
BTW - I generally found this problem on after a "halt". Reboots seem to work fine.
